Sloan brings to her Care Manager role, experience in patient-centered healthcare. She has been a Registered Nurse for over 30 years. Her compassionate nature and passion for helping others enable her to thrive in various settings. After working in pediatrics for many years, she transitioned to working with the senior population as a nurse supervisor in a home care company. 

As a care manager with Preferred Living Solutions, Sloan’s warm personality and exceptional ability to engage with others accelerate a trust that is crucial for developing a superb relationship with her clients. She is well-equipped to come alongside families and provide the support needed to help them or their loved ones remain as independent as possible, keeping safety and risk factors in mind. Her knowledge of providing assessments, creating individualized care plans, managing caregivers, and providing accurate communication, help her to provide exceptional care management for her clients.   

Sloan enjoys spending her free time hanging out with family, traveling, playing outdoors, and volunteering at Saving Grace non-profit dog rescue.
